Philly Moves
Event Agreement

By attending The Movement Foundation, Inc. Philly Moves events, I hereby acknowledge and agree that (a) there is risk of serious injury from activities involved, (b) I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my participation, and (c) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, covenant not to sue, discharge, and hold harmless The Movement Foundation, Inc., their contractors, volunteers, agents, and owners of premises used to conduct The Movement Foundation, Inc. Philly Moves event, of and from any injury, harm, disability or loss I may suffer, including death, whether arising from the negligence of the releasees or otherwise.

I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by participation in The Movement Foundation, Inc. Philly Moves event; and I acknowledge that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID- 19 by participation in The Movement Foundation, Inc. events or activities may result from the actions, omissions, or negligence of myself and others, including, but not limited to, The Movement Foundation, Inc. contractors, volunteers, agents, and other class participants.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself, illness, damage, loss, claim, liability, or expense of any kind, that I may experience or incur in connection with my participation in The Movement Foundation, Inc. Philly Moves event (collectively, the “Claims”). On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless The Movement Foundation, Inc., its contractors, volunteers, and agents, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of The Movement Foundation, Inc., its contractors, volunteers, or agents, whether a COVID-19 infection occurs before, during, or after my participation in The Movement Foundation, Inc. Philly Moves event.

I agree that I will not attend The Movement Foundation, Inc. Philly Moves event if I am afflicted with, or have knowingly, within the last 14 days, been in contact with someone afflicted with, COVID-19, or if (ii) I am experiencing a fever, or signs of respiratory illness such as cough, shortness of breath or difficulty breathing, or other COVID-19 symptoms.
